In consideration of being allowed to participate in the event or activity referenced above, I acknowledge, appreciate, and agree that:

Coach's/Director's Acknowledgments:

Registration for this event is not completed until payment has been received for the tournament. Payment may be made online, or by mail. Should I complete payment after the tournament is full, I understand my team is placed on a waiting list and admitted first come, first served. Team space will not be held until payment has been received and the online registration form has been completed. I agree to complete in full all tournament rosters and participant waivers in a timely manner based on the event website.

Tournament Waiver of Liability:

• I assume all risks and the risks of my team from the participation in this tournament, and will hold harmless MCC Sports, Inc., Peachtree City County Park and Recreation, and all of the named entities affiliates, associates, volunteers, and employees, of any and all liability, actions, causes of action, claims and demands of every kind and nature whatsoever which may arise in connection with or resulting from participation in the tournament or related activities, whether the result of negligence or any other cause.
• I certify to follow the tournament entry rules and deadlines and recognize that if I do not meet the deadlines and due dates of the tournament that my team space (s) will be forfeited. I agree to conduct myself and hold my team to a high degree of sportsmanship and recognize that failing to honor the game will result in disciplinary action by the tournament officials or director. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
• I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
• I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
• I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Releasees, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
